Job summary

Kaleidoscope Family Solutions ABA, Inc. in Bryn Mawr, PA seeks an IBHS Case Manager to oversee care coordination for children and families in the IBHS program.

The role focuses on managing health and proactive relationships with families and school communities. You will work with clinicians, CBH, schools, and providers to ensure timely services, maintain caseloads, and ensure accurate documentation in the electronic health record.

Qualifications

• Bachelor's degree required.
• Minimum of three (3) years' case management, counseling, or direct care mental health experience with children and families.
• Knowledge of IBHS regulations a plus.
• Reliable transportation.

Responsibilities

• Assess the service needs based on a Physician/Psychologist evaluation by completing the IBHS intake.
• Identify, link, and coordinate services including preparation and distribution of IBHS packets and starting up new children.
• Ensure children are CBH eligible to participate in IBHS.
• Facilitate access to services, coordinate with the clinical and recruiting teams.
• Maintain a schedule of key deadlines for each case.
• Work with clinical team to monitor the effectiveness of services. Reassess service needs.
• Maintain appropriate documentation of all care coordination/case management services provided in the electronic health record.
• Meet with the client and responsible parent within established parameter of the initial contact and maintain follow-up meetings weekly.
• Establish and maintain coordination with CBH, schools, day care, providers, or other child serving systems to ensure coordination of care.
• Work collaboratively with the clinical treatment team (BCBA, LBS, BHT) to ensure services are coordinated.
• Coordinate weekly service schedules of assigned providers.
• Support the collection and verification of weekly services hours and documentation for appropriate billing.
• Assume other duties assigned by Clinical Supervisor/Lead Clinician and/or Team Leader

Position Description

The IBHS Case Manager position is an integral part in Kaleidoscope ABA's mission to deliver therapeutic services to children with autism. This role's primary responsibility is to manage the health and proactive relationship with children, families, and school communities serviced under Kaleidoscope ABA IBHS program. The IBHS Case Manager will work to maintain a census/caseload that meets the standards as set in the program budget to provide care coordination support to children and families in the program.

Requirements

• Bachelor's degree in social work, Social Science, Bachelor of Science in Nursing, or Bachelor of Arts Degree from an accredited educational institution.
• Minimum of three (3) years' case management, counseling, or direct care mental health experience with children and families.
