You will assess needs, develop service plans, connect clients to resources, and advocate on their behalf.

Responsibilities include conducting assessments, updating care plans, coordinating resources, ensuring confidentiality, and collaborating with internal teams to deliver coordinated care.

Qualifikationen

• Bachelor’s degree in social work or related field, required.
• 1 year of work experience in a shelter, social services or nonprofit setting, required.
• CPR, BLS and First Aid certificates to be completed within three months of hire.
• Bilingual (Spanish/English), preferred.

• Conduct comprehensive client assessments to identify needs, strengths, barriers, and eligibility for services.
• Develop, implement, and update individualized service plans with clients and teams.
• Coordinate access to community resources, benefits, and support services.
• Provide ongoing case monitoring, follow-up, and progress toward goals.
• Engage in regular client contacts through visits or telephonic support.
• Advocate for clients to ensure equitable access to services.
