St. Joseph Center in Los Angeles is seeking a Case Manager I to support older adults with housing navigation, service referrals, and trauma-informed care. You will collaborate with social service staff to ensure quality client outcomes.

The role requires strong communication, organizational skills, and a willingness to work flexible hours, including some early mornings and evenings. A bachelor’s in social work or equivalent experience is expected.

Qualifications

• Excellent communication and interpersonal skills.
• Experience with housing programs and homeless services.
• Knowledge of mental health and substance use services.
• Bilingual background is a plus.
• Flexibility to work early mornings and evenings.
• 1+ year of experience serving individuals with serious mental illness or co-occurring disorders.
• Bachelor degree or 2+ years of related case management experience.

Responsabilités

• Provide ongoing case management services to individuals and families to assist them with retaining housing and be good tenants and neighbors.
• Provide appropriate referrals and linkages to programs and services to assist with increasing self-sufficiency.
• Develop close working relationships with outreach workers, case managers, police and paramedics, housing authorities, and partner agencies.
• Conduct initial mental health/substance abuse screenings and housing needs assessments.
• Develop and implement individualized care plans coordinating referrals and services addressing housing, mental health, relapse prevention, medications, and risk factors.
• Keep organized files for each client and enter data into the electronic database.
• Utilize Motivational Interviewing, Harm Reduction, Housing First, Trauma-Informed Care, and integrated treatment methodologies.
• Provide ongoing case management to ensure permanent housing, reduce isolation, prevent relapses, and advocate against evictions; link to medical/dental, mental health, substance abuse, life skills, and job services.
