Seven Hills Foundation is seeking a Case Manager I to join our Children’s Congregate Care team and provide essential support to youth in residential settings. You’ll coordinate services, support treatment goals, and assist with discharge planning and aftercare to promote stability and successful community reintegration.

The role requires strong communication, organization, and trauma-informed, client-centered care.

Qualifications

• Bachelor's degree preferred.
• Experience in behavioral health or residential settings is a plus.
• Strong communication and organizational skills required.

Seven Hills Foundation is seeking a Case Manager I to join our Children’s Congregate Care team and provide essential support to youth in residential settings.In this role, you’ll be part of a multidisciplinary team working to improve the mental health and overall well-being of children and adolescents.
