Job summary

The Salvation Army USA Western Territory is seeking a Full Time Veterans Services Case Manager at Owens Campus. You will conduct intake, assessments, and initial service planning, providing housing placement referrals and direct case management for veterans and their families.

Responsibilities include HMIS/SQUARES data entry, confidentiality, coordination with VA personnel, and outreach to local communities.

Qualifications

• Bachelor's degree in a Human Services discipline; 4 years of related experience may substitute for degree.
• Experience working with homeless and/or at‑risk veterans and families preferred.
• Strong oral and written communication skills; proficiency with Microsoft Office software.
• Must have a valid driver's license and a clean driving record; veteran status preferred.

Qualifications

• Bachelor's degree in a Human Services discipline. A minimum of 4 years of related employment experience may be substituted for degree requirements.
• Experience working with homeless and/ or at-risk veterans and families preferred.
• Strong oral and written communication skills.
• Proficiency in Microsoft Office Software (Word, Excel, etc.)
• Ability to communicate and work effectively with a diverse group of residents, staff, and community partners.
• Must have a valid driver's license and a clean driving record.
• Veteran status preferred
• Must be able to successfully pass a criminal background check.

Responsibilities

• Manage up to 15 cases
• Conduct intake, assessments, and initial service planning
• Provide referrals and support for housing placement
• Enter data in HMIS/SQUARES and ensure documentation compliance
• Conduct outreach in low-risk communities such as Summerlin, Centennial Hills, and Henderson
• Provide direct case management services to homeless and at-risk veterans and their families.
• Screen veterans' eligibility for the program and facilitate the intake process.
• Coordinate care with VA personnel and others on behalf of participants for eligibility
• documentation, mental and physical health services.
• Learn and maintain knowledge of community employment agencies and procedures, along with
• modifications to those procedures.
• Assist veterans in identifying education and training goals and developing plans to meet those goals.
• Assist veterans to identify mainstream needs and facilitate the process with appropriate service
• providers to meet those needs.
• Assist veterans and their families to alleviate employment barriers.
• Maintain, record, and report the progression of veterans and their families through program
• participation.
• Perform interventions and crisis-oriented counseling.
• Conduct outreach activities, including visits to facilities such as shelters, treatment programs,
• soup kitchens, and other locations where homeless and at-risk families may congregate.
• Conduct home visits to provide case management services.
• Maintain client databases and referrals for services and generate reports.
• Complete accurate and timely data entry into the HMIS system.
• Always maintain participant confidentiality.
• Attend all scheduled team meetings and training sessions.
• Other duties and special projects as assigned.
