Qualifications

• Bachelor’s degree in social work, human services, or related social science field.
• Valid driver’s license with acceptable driving record and a reliable vehicle.
• Successful background checks is required.
• Internet access at home required.

Responsibilities

• Assess the areas of the consumer’s life where assistance is needed.
• Coordinate resources and services for the person.
• Monitor service delivery and ensure needs are met with providers.
• Provide person-centered service planning and Coordinated Consumer Support Plans.
• Attend meetings with the person and their interdisciplinary teams.
• Travel to different sites in various counties for meetings.
• Manage casework for EW, AC, RULE 185, CADI, CAC, and IDD waivers.
