The Case Manager provides comprehensive case management services to individuals and families at risk of homelessness. Responsibilities include conducting assessments, developing service plans, connecting participants to housing and community resources, assisting with benefits and financial assistance applications, and supporting clients in achieving long-term housing stability and self-sufficiency.
