Job summary

Joyous Ventures is seeking a compassionate Case Manager to support justice-impacted individuals facing homelessness or housing instability in the Los Angeles area. The role involves coordinating resources and addressing medical, mental health, and substance use barriers to promote long-term stability.

The position is fast-paced and participant-facing, requiring patience and comfort working with individuals experiencing homelessness, mental illness, substance use, and domestic violence.

Qualifications

• Bachelor's degree in Social Work or related field required.
• Experience with homelessness and/or justice-involved individuals preferred.
• Knowledge of harm reduction, trauma-informed care, and motivational interviewing.
• California driver's license and valid insurance.
• Physical ability to stand/walk long periods.
• BLS/CPR certification required.

Responsibilities

• Conduct intake assessments to determine housing and health needs.
• Develop and update ISPs based on progress.
• Connect participants to community services including housing and health.
• Accompany participants to appointments.
• Advocate for access to care and services.
• Document case notes and track progress for reports.
• Participate in weekly case conferences and trainings.
• Maintain confidentiality and HIPAA compliance.
• Coordinate with external providers for housing retention.
