Responsibilities include case management, housing navigation, and ongoing documentation to meet licensing and grant requirements, while supporting client outcomes and community partnerships.

Qualifications

• Bachelor's degree in social work, psychology, human services, or related field.
• Ability to work effectively with Clarity HMIS database and ensure accurate data entry.
• Strong interpersonal communication and sensitivity to diverse clientele.

Responsibilities

• Provides case management, housing navigation, case meetings, and case plan oversight.
• Maintains timely, accurate records in client databases and notes in accordance with policy and law.
• Ensures high-quality opportunities for client progress in housing, parenting, education, employment, and stability.
