Job summary

Portage County Health & Human Services is hiring a Case Manager to support the Portage House. This is a full-time position (40 hrs/week), Monday through Friday, with an application deadline of 2026-09-18. The role includes client case management, counseling, house supervision, and coordination with DCC agents.

Qualifications

• High school diploma or equivalent required; two years of post-secondary education in related field or equivalent experience.
• Group and individual case management experience required.
• Must possess a valid driver’s license and travel as needed for work.

Responsibilities

• Provide case management for identified clients.
• Conduct weekly individual counseling sessions and document notes.
• Facilitate educational modules and house meetings.
• Attend staff meetings and supervise clients.
• Respond to policy infractions with disciplinary reports and actions.
• Record client activity and prepare client reports as assigned.
• Coordinate case planning with DCC agents and other service providers.
• Provide house supervision and ensure safety and rule adherence.
