Job summary

CAMBA is hiring a Case Manager to assess needs, develop service plans, and coordinate access to housing, benefits, health care and education services for clients. You will conduct interviews, document progress, and advocate with outside organizations to secure community resources.

Requirements include an associate degree with relevant experience or a high school diploma with four years of case management work. Proficiency in Microsoft Office and strong writing/communication skills are essential.

Qualifications

• Associate degree with 2 years relevant experience, or high school diploma with 4 years case management experience.
• Proficiency in Microsoft Office (Excel, Word, Outlook).
• Excellent writing and communication skills for assessments and reports.

Responsibilities

• Prescreen clients for eligibility and schedule intake appointments.
• Input client data and progress notes into automated databases.
• Develop service plans and follow up to monitor progress and outcomes.
• Refer clients to appropriate services and assist with benefits applications.
• Escort clients to appointments and maintain compliant documentation.
