Job summary

S:US is seeking a Case Manager for their homeless prevention program, split between West 181st Street and Rikers Island locations in New York., with onsite presence required at least two days weekly and ongoing travel between sites.

The role focuses on comprehensive case management, housing resource access, service coordination, and long-term housing stability for individuals and families at risk of homelessness.

Qualifications

• HS Diploma, GED, or AA degree in related field with two years qualifying experience.
• MSW or bachelor’s degree preferred in related field.
• Knowledge of the Department of Homeless Services, the Human Resources Administration, and entitlements.
• Understanding homelessness, incarceration, substance abuse, mental health and domestic violence with cultural sensitivity.

Responsibilities

• Provide case management to individuals and families at risk of homelessness.
• Conduct assessments and develop service plans to address housing barriers.
• Connect participants to housing resources, benefits, and services.
• Assist clients with financial assistance applications and benefits enrollment.
• Advocate and coordinate services with landlords and community providers.
• Monitor progress and maintain accurate case records and documentation.
• Support housing stability, self-sufficiency and long-term success.
• Collaborate with internal and external partners to ensure comprehensive services.
• Other duties as assigned.
