Job summary

S:US is seeking a Case Manager for their homeless prevention program located at W. 181 Street, New York, NY.

The role focuses on conducting assessments, developing service plans, coordinating housing resources, and assisting with benefits to support long-term housing stability. Responsibilities include advocating for clients, documenting progress, and collaborating with partners to deliver comprehensive services.

Qualifications

• HS Diploma, GED, or AA degree in related field and two years qualifying experience.
• MSW or bachelor’s degree preferred in related field.
• Knowledge of the Department of Homeless Services, the Human Resources Administration, and entitlements.
• Knowledge of the complex interaction of homelessness, incarceration, substance abuse, mental health and domestic violence is preferred with sensitivity to cultural and gender specific issues with both individuals and families.

Responsibilities

• Provide case management services to individuals and families at risk of homelessness.
• Conduct assessments and develop individualized service plans to address housing barriers.
• Connect participants to housing resources, benefits, and supportive services.
• Assist clients with financial assistance applications and benefit enrollment or reinstatement.
• Advocate for clients and coordinate services with community providers and landlords.
• Monitor participant progress and maintain accurate case records and documentation.
• Support clients in achieving housing stability, self-sufficiency, and long-term success.
• Collaborate with internal and external partners to ensure participants receive comprehensive services.
• Other duties as assigned.
