Job summary

S:US is seeking a Case Manager for the Homebase program in Manhattan to support eviction prevention and housing stability. You will deliver full case management, connect participants to benefits, subsidies, legal aid, and Homebase services, and assist tenants at the housing court to resolve housing crises.

Qualifications

• HS Diploma, GED, or AA degree in related field with two years qualifying experience.
• Ability to work in the field and/or Homebase office up to five days per week.
• Must be able to travel to housing court and HRA/DSS offices up to two days per week.
• Proficiency in database/client record systems data entry and in Microsoft 365.
• Bachelor’s degree preferred in related field.
• Bi-lingual English/Spanish speaking applicants a plus.
• Knowledge of the Department of Homeless Services, the Human Resources Administration, and entitlements.

Responsibilities

• Oversee case management for program participants and families.
• Ensure community follow-up to engage participants in care and empower self-sufficiency.
• Participate in outreach at Manhattan housing court and HR offices.
• Promote linkage with service providers to support housing stability and legal aid.
• Monitor linkages with on-island service providers.
• Prepare and submit Financial Assistance Requests for participants.
• Coordinate housing assistance service planning and after-care.
