Job summary

CAMBA is seeking a Case Manager at our Staten Island location to support families and individuals with social services, housing, benefits, and education access. You will conduct interviews, assess needs, and coordinate service plans with partners to promote stability and self-sufficiency.

Responsibilities include prescreening for eligibility, documenting progress, and working to meet funding guidelines. A strong write-up of assessments and effective communication are essential to success.

Qualifications

• Bachelor's degree in social work or related field, or equivalent experience.
• Associate degree and 2 years of relevant experience.
• High School Diploma and 4 years of case management experience in social services.

Responsibilities

• Aid individuals and/or families requiring social service assistance.
• Conduct client interviews and evaluations to assess needs and develop service plans.
• Assist clients in identifying and locating community resources.
• Prescreen clients for eligibility and schedule intake appointments.
• Review documentation establishing clients' eligibility and conduct intake/ reassessments.
• Input client data into automated database.
• Create and maintain client files.
• Develop service plans tailored to client needs and provide ongoing follow-up.
