Clinica Sierra Vista in Bakersfield is seeking a Case Manager-HCH to join the Healthcare for the Homeless and Street Medicine teams. The role focuses on outreach to homeless individuals across encampments, parks, and service sites, and connects them with health care enrollment and social services.

You will promote opioid treatment access, provide logistics for housing and insurance, and document outreach activities while maintaining compassion and confidentiality.

Competenze

• Education: High school diploma or GED.
• Experience working with homeless populations and diverse cultures.
• Bilingual ability preferred.
• Proficient PC skills: MS Office, Access and Excel.
• Excellent verbal and written communication.
• Ability to manage multiple priorities in a fast-paced environment.
• Must pass criminal background clearance.
• Valid California driver’s license with reliable vehicle.
• TB clearance and ability to lift up to 50 pounds.
