Job summary

The team coordinates care, conducts wellness and risk assessments, and links clients to housing and services while maintaining documentation and collaborating with city and partner staff.

Qualifications

• High School degree or equivalent plus 2 years or more TARGET EXPERIENCE, or
• Specialty Certificate related to the populations served and 1 year TARGET EXPERIENCE

Responsibilities

• Conduct care coordination for people experiencing homelessness as directed by a Neighborhood Street Team lead.
• Engage individuals with complex medical and/or behavioral issues and assess needs.
• Initiate referrals and linkages, participate in treatment planning, and stabilize into permanent housing.
• Develop engagement strategies and interventions; follow up with clients to support linkage.
• Maintain timely and accurate documentation according to program requirements.
