Job summary

PAM Health Specialty Hospital of Heritage Valley is seeking a Case Manager to coordinate interdisciplinary care for a designated patient population within an inpatient setting.

You will review admission records, assess discharge needs, and coordinate discharge planning while working with the interdisciplinary team to improve financial and quality outcomes and strengthen payor relationships.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or licensed social worker.
• Current BLS certification required.

Responsibilities

• The Case Manager is responsible for the interdisciplinary coordination of care for a designated patient population.
• Reviews inpatient admission records to ensure proper utilization and admission decisions.
• Assess discharge needs and coordinate discharge plan.
• Work collaboratively with the interdisciplinary team to achieve financial and quality outcomes.
• Maintain and enhance payor relationships.
