Job summary

Northern Kentucky Area Development District is seeking a Case Manager to provide assessment and case management for Participant Directed Services (PDS) Waiver participants in Florence. The role involves evaluating eligibility, developing person-centered care plans, and monitoring services to meet participant needs.

The position requires strong communication, HIPAA knowledge, and the ability to travel for home visits across NKADD counties. Office-based in Florence with field visits as needed.

Qualifications

  • At least one year of relevant experience in medical social work, geriatrics, or with diverse populations.

Responsibilities

  • Conduct eligibility assessments and ongoing care management for participants in line with program policies and timelines.
  • Maintain a caseload and complete home visits to assess eligibility and participant needs.
  • Develop, update, and monitor individualized plans of care with participants and caregivers.
  • Ensure accurate authorization and billing of services; monitor delivery and resolve issues.

Job description

Position Title: Case Manager
Division: Aging and Disability Services
Work Location: Florence
Exempt Status: Salary Non- Exempt
Supervisor: Waiver Supervisor
Supervises: N/A
Position Purpose:

This is specialized and technical work in providing assessment and case management services to Participant Directed Services (PDS) Waiver participants, which includes ongoing assessment for program eligibility and monitoring all aspects of care and consumer services to ensure needs of participant are being met. The role involves evaluating eligibility, developing person-centered care plans, and monitoring services to ensure participant needs are met. Work is performed under moderate to limited supervision.

Essential Duties & Responsibilities:
  • Conduct eligibility assessments and ongoing care management for assigned participants, in compliance with program policies and timelines.
  • Maintain a caseload and complete home visits to assess initial and continued program eligibility and participant needs.
  • Develop, update, and monitor individualized plans of care in collaboration with participants and caregivers.
  • Ensure accurate authorization and billing of services; monitor service delivery and resolve issues as needed.
  • Review electronic timesheets and collaborate with the PDS Financial Management Agency (FMA) on any payment concerns.
  • Serve as liaison and advocate between participants, providers, caregivers, and other stakeholders.
  • Participate in case conferences, staff meetings, and professional trainings as required by DAIL and NKADD.
  • Document all contacts, assessments, and actions in the approved case management system and maintain accurate client files.
  • Provide on-call phone coverage as scheduled.
  • Represent the program and agency professionally in all communications and interactions.
Secondary Responsibilities:
  • Assist with agency-wide projects, committees, and special initiatives as assigned.
  • Provide coverage for other Care Managers/Brokers as needed.
  • Perform additional duties as assigned.
Minimum Qualifications:
  • Education & Credentials:
    • Bachelor's degree in a Human Services field, in good standing with any applicable credentialing board or Registered Nurse (licensed in Kentucky).
  • Experience:
    • At least one year of relevant experience in medical social work, geriatrics, or working with diverse populations across the lifespan.
Knowledge, Skills & Abilities:
  • Strong interpersonal skills with the ability to communicate effectively across diverse populations.
  • Working knowledge of HIPAA and other confidentiality regulations.
  • Proficient in using computers and data entry systems; ability to learn multiple program-specific tools.
  • Solid understanding of community, state, and federal resources.
  • Strong documentation, organizational, and time-management skills.
  • Ability to make sound judgments and manage complex, sometimes stressful situations.
  • Willingness and ability to travel throughout NKADD counties for home visits and meetings.
Special Requirements:
  • Valid driver’s license and reliable transportation.
  • Ability to independently enter/exit vehicle and conduct in-person visits in a range of home environments, which may present environmental and physical hazards.
Work Environment & Physical Requirements:
  • Office-based work in a standard cubicle setting, involving frequent computer and phone use.
  • Requires standing, sitting, and occasional stooping.
  • Regular home visitations required; must be able to safely and effectively perform visits in various home settings.
