Job summary

Job description

Performs all activities related to case management services to achieve program goals at main office and at clients’ residences that may or may not be accessible by public transportation. Serves as an advocate for the client population.

Provide case management services to assigned clients on an ongoing and emergency basis according to State of Indiana or other applicable guidelines, by telephone or in clients’ residences that may or may not be accessible by public transportation.

Work within an active phone loop system to provide information and referral services on an ongoing basis.

Develop and maintain an accurate and thorough knowledge of all program requirements.

Attain within initial timeframe and maintain annually Case Management Certification according to State requirements.

Develop and maintain skillset on tools required to perform job tasks in timely and accurate manner, including specialized computer software.

Develop and maintain accurate knowledge of, and monitor compliance with, HIPAA and agency confidentiality policies.

Organize and document daily schedule to inform supervisor of activities, and adhere to safety protocol.

Demonstrate competency by meeting or exceeding department work performance requirements.

Serve as an advocate for the client population, including compliance with State incident reporting requirements, and adult and child protective service referrals.

Communicate with supervisor on a daily basis, and as needed, to report all areas of concern/issues/incidents.

Serve as an example of agency Standard of Excellence by exemplifying agency vision and goals.

Demonstrate professionalism in relationships with customers/co-workers/supervisors to build rapport while refraining from inappropriately familiar personal relationships.

Complete and attest to accuracy of payroll records, and submit for review and processing according to agency schedule.

Comply with HIPAA and agency confidentiality policies and report any potential issues immediately.

Develop and maintain knowledge of community resources available to assist clients; work with other departments to acquire knowledge of community resources available.

Develop and maintain an accurate and thorough knowledge of hardware platforms and software applications used to communicate effectively with clients. Must be able to effectively use telehealth/technology implemented for virtual communication.

Required Education and Experience

Bachelor’s degree from four-year college or university.

Preferred Education and Experience

Internship or practicum in related field as part of bachelor’s degree requirement from four-year college or university. Two years full-time, direct service experience with the elderly or disabled (includes assessment, care plan development and monitoring); or a Registered Nurse with one year’s experience in human services; or a master’s degree in a related field may be a substitute for the required experience.

Additional Eligibility Requirements

Must successfully complete and annually maintain Division of Aging Case Management Certification. Must possess and provide annually valid driver’s license and documentation of current automobile insurance at agency-specified levels. Must provide negative TB certificate, or documentation to support exemption, annually. Must comply with requirements of other facilities (hospitals, LTC facilities, etc.) related to flu or other vaccines if job duties require providing services in those facilities.

Expected Hours of Work

This position is a non-exempt position and may be full‑time or part‑time depending on the agency’s needs. Agency business hours are typically Monday through Friday, 8:00am–4:30pm.

Travel

Travel is throughout the agency’s service area during agency business hours.

Physical Demands

The individual must be able to communicate effectively with others and enable understanding by others through communication. The individual is required to sit and/or stand for long periods of time. The individual is required to transport, safeguard, and use a laptop computer and client files.

Work Environment

This job operates in a professional office environment using standard office equipment such as computers, phones, photocopiers, fax machines, etc. This job also operates at clients’ residences.
