Job summary

Northern Kentucky Area Development District in Florence seeks a Case Manager to provide assessment and ongoing case management for Participant Directed Services (PDS) Waiver participants. The role includes eligibility evaluation, care planning, and monitoring services to meet participant needs.

The position requires home visits, collaboration with participants, caregivers, and providers, and accurate documentation in the case management system. Travel within NKADD counties is expected.

Qualifications

• Bachelor's degree in a Human Services field or RN license in Kentucky.
• At least one year of medical social work, geriatrics, or related experience.
• Strong interpersonal skills and ability to communicate across diverse populations.
• Working knowledge of HIPAA and confidentiality regulations.
• Proficiency with computers and data entry; ability to learn program tools.

Responsibilities

• Conduct eligibility assessments and ongoing care management for participants.
• Maintain caseload and complete home visits to assess eligibility and needs.
• Develop, update, and monitor individualized plans of care with participants and caregivers.
• Ensure accurate authorization and billing; monitor service delivery.
• Review electronic timesheets and coordinate with PDS Financial Management Agency on payments.
• Serve as liaison between participants, providers, caregivers, and stakeholders.
• Participate in case conferences, staff meetings, and trainings as required.
• Document contacts and actions in the case management system; maintain files.
• Provide on-call coverage as scheduled.
• Represent the program and agency professionally in communications.
