Job summary

Universal Health Services, Inc. (UHS) in partnership with Greystone Foster Care seeks a dedicated Case Manager for the Foster Care Program. This hybrid role blends remote work with in-office visits, requiring full-time commitment and occasional weekend/evening coverage in Tennessee.

You will coordinate placement documentation, audits, parent reassessments, and team meetings while ensuring compliance with state regulations and service plans. A strong background in social work is preferred.

Qualifications

• Bachelor's degree in Social Work or related field; Master's preferred.
• 1–3 years of related experience or training.
• Valid state driver's license required.
• Proof of automobile liability insurance required.

Responsibilities

• Complies with state regulatory requirements and Greystone Foster Care plan.
• Ensures that all placement documentation is completed within established timelines.
• Performs regular file audits and participates in peer audits for audit readiness.
• Assists with foster parent reassessments as needed.
• Schedules quarterly child/family team meetings; submits documentation.
• Visits each assigned child as per policy or as needed.
• Ensures EPSDT and dental appointments are scheduled and communicated to DCS.
• Ensures outpatient mental health assessments are scheduled for Level 2 youth.
• Obtains consents and releases from guardians for medical needs.

Case Manager - Foster Care Program

The ideal candidate must reside in Tullahoma or Manchester, Coffee County, Tennessee.

Universal Health Services, Inc. company, in partnership with Greystone Foster Care, has a long history in providing comprehensive behavioral health care services to at-risk youth struggling with extensive mental health issues. Greystone Foster Care recruits foster parents and offers the training to be equipped to provide a safe, stable and therapeutic environment for children and youth that have been removed from their homes. Our training prepares staff and foster parents to support children and youth as they begin the healing process. Our therapeutic foster parents link children and youth to hope, healing and trust.

Greystone Foster Care is seeking a dedicated, compassionate Case Manager to add to our growing team. This is a hybrid job blends remote work and in-office position with occasional requirements to report to the facility. This role requires full-time dedication and must be flexible to include weekends and evening work.

Job Duties/Responsibilities

• Complies with state regulatory requirements and Greystone Foster Care plan
• Ensures that all placement documentation is completed according to established timelines
• Completes regular file audits for each child and participates in regular peer file audits to ensure audit preparedness at all times
• Assists with completing foster parent reassessments when necessary
• Schedules child/family team meetings at least quarterly and corresponding documentation is completed/submitted
• Visit each assigned child per policy or as needed
• Ensures that annual EPSDT and biannual dental appointments are scheduled and information provided to DCS
• Ensures that outpatient mental health assessments are scheduled for all Level 2 youth and participation in recommended services occurs
• Obtains required consents and releases from appointed guardians for medical appointments and medications
