Job summary

NewPath is seeking a Foster Care Case Manager to support children and families in achieving goals within our Foster Care Program.

The role involves conducting individual, family, and group interventions, coordinating assessments, and ensuring accurate documentation while delivering culturally informed services in the Cincinnati area.

Qualifications

• HS diploma or GED with 5 years' experience working with SED children, OR a bachelor's degree with at least 6 months experience with SED children.
• Excellent written and verbal communication skills.
• Ability to work flexible hours, including some evenings and weekends.
• Initiative and ability to work independently; willingness to provide community-based services to a diverse population.

Responsibilities

• Conduct individual, family and group treatment interventions as indicated in the ISP.
• Coordinate evaluations and assessments (DLA-20, ISP, PHQ9, Ohio Scales) to identify client needs.
• Provide transportation as needed to reach goals.
• Maintain professional relationships and deliver culturally informed services.
• Adhere to agency policies, safety procedures and documentation standards.

The NewPath Child & Family Solutions Foster Care Case Manager is responsible for assisting children and families in achieving stated goals and objectives and maximizing independence and daily functioning through interventions and advocacy in our Foster Care Program.

Location:

This position is based at our NewPath Central Campus, 4721 Reading Rd. Cincinnati, OH 45237 and will work with NewPath clients within Hamilton County, Butler County & Montgomery County.

Your Role:

• Conduct individual, family and group treatment interventions as indicated in the Individualized Service Plan.
• Coordinate the necessary evaluations and assessments in order to identify each client's needs, DLA-20, ISP, PHQ9, Ohio Scales etc.
• Provide transportation as appropriate and necessary to reach identified goals.
• Maintain and model appropriate interpersonal relationships.
• Deliver culturally informed services.
• Maintain professional objectivity and self-discipline.
• Adhere to agency risk assessments, policies, protocols, safety questionnaire, on‑call procedure, safety planning, and corporate compliance.
• Ensure that required elements of the ECR/EHR are present, current and in accordance with agency certification and documentation standards.

Qualifications:

• Minimum of High School diploma and 5 years' experience with SED children; OR a Bachelor's degree in relevant field and at least 6 months experience working with SED children.
• Excellent written and verbal skills.
• Ability to work flexible hours, including some evenings and weekends.
• Initiative and ability to work independently;
• A willingness and comfort level in providing community‑based services to a diverse population of clients;
• Excellent problem-solving skills and the ability to remain calm and professional in crisis situation.
• Must be a minimum of 21 years of age, have a valid driver's license and proof of current auto insurance with no more than 5 points on your MVR.
• Must be able to pass a pre‑employment background check & drug screen.
