Job summary

NewPath Child & Family Solutions is seeking a School Based Case Manager to support children and families within the New Miami School District in Butler County. The home base is the NewPath Butler Campus in Hamilton, OH, with direct service through schools and community-based outreach.

You will coordinate assessments, deliver interventions, and advocate for client goals while maintaining professional conduct and compliance with agency policies.

Qualifications

• Demonstrated strong written and verbal communication.
• Ability to work flexible hours, including some evenings and weekends.
• Willingness to provide community-based services in diverse populations.
• Must be 21+ with valid driver's license and insurance.

Responsibilities

• Conduct individual, family and group treatment interventions as indicated in the ISP.
• Coordinate evaluations and assessments (e.g., DLA-20, ISP, PHQ9, Ohio Scales).
• Provide transportation to reach identified goals when needed.
• Maintain professional relationships and culturally informed services.
• Ensure ECR/EHR documentation is current and compliant.

The NewPath Child & Family Solutions School Based Case Manager is responsible for assisting children and families in achieving stated goals and objectives and maximizing independence and daily functioning through interventions and advocacy through our partnership with New Miami School District.

Location:

This position's home base is at our NewPath Butler Campus, 3570 Pleasant Ave, Hamilton, OH 45015 and will work directly within New Miami Schools in Butler County.

Your Role:

• Conduct individual, family and group treatment interventions as indicated in the Individualized Service Plan.
• Coordinate the necessary evaluations and assessments in order to identify each client's needs, DLA-20, ISP, PHQ9, Ohio Scales etc.
• Provide transportation as appropriate and necessary to reach identified goals.
• Maintain and model appropriate interpersonal relationships.
• Deliver culturally informed services.
• Maintain professional objectivity and self-discipline.
• Adhere to agency risk assessments, policies, protocols, safety questionnaire, on-call procedure, safety planning, and corporate compliance.
• Ensure that required elements of the ECR/EHR are present, current and in accordance with agency certification and documentation standards.

Qualifications:

• Minimum of High School diploma and 5 years' experience with SED children; OR a Bachelor's degree in relevant field and at least 6 months experience working with SED children.
• Excellent written and verbal skills.
• Ability to work flexible hours, including some evenings and weekends.
• Initiative and ability to work independently;
• A willingness and comfort level in providing community-based services to a diverse population of clients;
• Excellent problem-solving skills and the ability to remain calm and professional in crisis situation.
• Must be a minimum of 21 years of age, have a valid driver's license and proof of current auto insurance with no more than 5 points on your MVR.
• Must be able to pass a pre-employment background check & drug screen.
