Indiana Professional Management Group in New Castle, IN is seeking a Full Time Case Manager to support Individuals through Medicaid waivers in a hybrid capacity. Travel to community-based meetings is required, with in-home office work and in-person meetings as needed, and applicants must reside in Indiana or within 10 miles of the state line.

Qualifications

• Bachelor's in psychology, sociology, social work, special education, nursing, gerontology, counseling or rehabilitation, or another human services field.
• Or a Bachelor's in an unrelated field with at least 1 year direct experience with individuals with intellectual/developmental disabilities, or 2 years with aging population.
• Valid driver's license and reliable transportation.
• Ability to pass background screens.
• Designated home office with computer, internet and smartphone.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on strengths, needs, goals and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

This position requires in-person meetings; therefore, applicants must live within the State of Indiana or within 10 miles of the state line.

What You’ll Do:

Who You’ll Work With:

• Case managers work with Individuals on four Medicaid Waivers; Family Supports Waiver (FSW), Community Integrated Habilitation Waiver (CIH), Health & Wellness Waiver (H&W), and Traumatic Brain Injury Waiver (TBI)

What You Need:

A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or

A Bachelor's degree in an unrelated fieldanda minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population.

A valid driver's license, car insurance, and reliable transportation

The ability to pass thorough background screens

Designated home office setting with computer, internet service, and smart phone
