Job summary

PAM Health Rehabilitation Hospital of Surprise is seeking a Case Manager to coordinate interdisciplinary care for an inpatient population. You will review admission records to ensure appropriate level of care and identify discharge needs.

You will work with the care team to optimize outcomes, arrange discharge planning, and maintain payor relationships. Licensure in the state and BLS are preferred, with three to five years of inpatient experience in acute, IRF, or LTACH settings.

Qualifications

• Licensure in the state where the hospital resides is preferred.
• Professional disciplines include RN, OT, PT, SLP or Social Worker with BLS certification.
• Three to five years of inpatient experience, preferably in acute, IRF, or LTACH.

Responsibilities

• Coordinate interdisciplinary care for designated inpatient population.
• Review admission records to ensure appropriate level of care.
• Assess discharge needs and coordinate discharge planning with the team.
• Maintain and enhance payor relationships.
