Job summary

PAM Health Specialty Hospital of Shreveport is seeking a Case Manager to coordinate care for an assigned patient population within an acute inpatient setting. You will review admission records to ensure appropriate level of care and collaborate with the interdisciplinary team to facilitate discharge planning and resource utilization.

Ideal candidates will have licensure in Louisiana, current BLS certification, and 3–5 years of inpatient experience, preferably in IRF/LTACH settings.

Qualifications

• Interdisciplinary coordination across care teams.
• Focus on discharge planning and appropriate utilization of resources.
• Experience in inpatient settings preferred.
• Payor relationship management and care coordination.

Responsibilities

• Coordinate interdisciplinary care for designated patient population.
• Review inpatient admission records for appropriate utilization and level of care.
• Identify discharge needs and coordinate the discharge plan with the team.
• Maintain and enhance payor relationships to support patient care and outcomes.
