Job summary

CAMBA in New York, NY is seeking a Case Manager to aid individuals and families requiring social service assistance, assess needs, and develop independent living plans. You will locate and connect tenants with appropriate community resources and document progress.

Full-time or part-time opportunities with flexible hours, offering health and dental insurance, retirement plan, paid time off and holidays. You'll work at 82-41 Parsons Blvd, Jamaica, NY, serving residents in the Jamaica/Queens area.

Qualifications

• Bachelor's degree and experience with the seriously and Persistently Mentally Ill (SPMI) population, and/or equivalent experience.

Responsibilities

• Aid individuals and/or families requiring social service assistance.
• Interview and evaluate tenants and formulate independent living plans.
• Locate and provide access to appropriate community resources for tenants.
• Review documentation establishing tenants' eligibility for program and make copies.
• Assist tenants in attaining their goals by identifying resources and making referrals.
• Monitor tenants' progress via phone or in-person visits and document progress notes.
• Participate in case conferences.
• May escort tenants to appointments and prepare marketing materials.
