Job summary

Canopy Children's Solutions in Mississippi seeks a Case Manager for the in-Circle Program. You will assess needs and link children and families to social, educational, vocational, and other wraparound services.

As an adult role model, you will work with families and CPS to implement service plans, monitor progress, and document outcomes. Must have a BA in Social Work and 2+ years' experience; valid driver's license and willingness to use personal vehicle.

Qualifications

• Bachelor’s degree in Social Work or related field and at least two years of experience.
• Proficient in completing documentation and reports related to services provided.
• Must have a valid driver’s license and willingness to use personal vehicle for organization business.

Responsibilities

• Assess client needs and link child and family with wrap around services.
• Collaborate with families to create a supportive environment addressing basic needs.
• Work with CPS caseworker to ensure effective implementation of services and adjustments as needed.
• Develop service plans, referrals and monitor progress and outcomes.
• Coordinate and monitor case management activities across home, school, and community settings.
