Job summary

SEMCOA is seeking a case manager to provide direct resident care, including assessments, counseling, discharge planning, and aftercare. The role requires travel and collaboration with team members to help clients secure housing and stability.

You will facilitate daily groups, maintain medical records, and coordinate with community resources to ensure comprehensive support for families and individuals in recovery.

Qualifications

• Associate's degree in human services, High School Diploma or GED preferred.
• One year of experience in substance use treatment and/or mental health field.
• CPR Certification preferred.
• Spanish or Haitian Creole speakers encouraged to apply.
• Valid Driver’s License, vehicle, and able to provide clean driving record.

Responsibilities

• Provide direct care to residents, including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and bio-psychosocial assessments.
• Make referrals when appropriate.
• Link residents with aftercare services prior to discharge and aid access to services for families as needed.
• Strengthen partnerships within the community to help clients achieve sobriety and housing stability.
• Review patient/family rights, emergency safety plans, and grievance policies; involve families in treatment as requested.
