Job summary

Marsell Wellness Center is seeking a Case Manager to coordinate targeted and general case work for families and children. The role emphasizes strength-based, needs-based, and community-based approaches, building networks to improve services.

The ideal candidate holds a Bachelor's degree with 1–2 years of related experience, and demonstrates analytical, communication, and organizational skills to develop care plans, conduct psychosocial assessments, and coordinate across providers.

Qualifications

• Bachelor's degree from a four-year college or university; and one to two years related experience and/or training.
• Strong reading, writing and communication skills for diverse stakeholders.
• Ability to analyze data, assess needs, and coordinate services.

Responsibilities

• Plan, train, and coach the case management work of a unit of trained case managers performing difficult or complex casework.
• Investigate and provide services and referrals for clients where welfare is involved.
• Work with cases involving neglect, abuse, or health conditions affecting family dynamics.
• Develop treatment plans to improve or restore family functioning.
• Make case studies to assess problems and determine intervention methods.
• Conduct thorough psychosocial assessments on families.
• Develop person-centered, strength-based case plans.
• Assess family environment to determine threats to child permanency and stability.
• Maintain up-to-date records and prepare reports and correspondence.
• Travel to include transportation of children to parents/sponsor on rotation with other positions.
• Perform other duties as requested by supervisor/management.
