Qualifications

• Bachelor's degree in a relevant field is required.
• Minimum of 2 years' experience in the social services field.
• Strong interpersonal and verbal/written communication skills.
• Must be organized.
• Experience with DHS CARES/ Homeless population is a plus.

Responsibilities

• Completes intake assessments and collects required documents.
• Maintains client records and progress notes.Conducts weekly interdisciplinary meetings and evaluations.
• Collaborates with Housing Specialist to create exit strategies and ILPs.
• Advocates for clients and makes referrals to housing and other service providers.
• Reports caseload summaries to Social Services Director and Case Manager Supervisor.
• Responds to emergencies as Essential Employee.
