Job summary

IVYPARK in is seeking a qualified nurse/care manager to coordinate resident care plans, conduct assessments, and liaise with families, providers, and payers. The role emphasizes clinical expertise, critical thinking, documentation, and efficient patient-centered care within a cost-conscious framework.

The position requires nursing or social work credentials, state licensure, and experience with Medicare/Medicaid programs.

Qualifications

• Bachelor's Degree in Nursing or Social Work required.
• RN license preferred; LVN/LPN acceptable.
• Two (2) years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-cal and Medicaid programs and benefits.
• PCC Knowledge preferred.
• Certificate as a CCM a plus.
• Must maintain continuing education/licensing.
• Must remain in good standing with the Department of Public Health, License and Certification Division.

Responsibilities

• Utilize clinical expertise and critical thinking skills to develop and implement a plan of care with residents and families.
• Communicate regularly with residents and family about Plans of Care, PT/OT and other treatment protocols.
• Attend morning meetings/stand up to facilitate team communications.
• Answer residents' questions about care, treatment plans and illness progression.
• Monitor and adjust residents' statuses based on changing needs and conditions.
• Organize and prioritize daily work by assessing residents' needs in area of responsibility.
• Complete documentation as required.
• Perform utilization review activities to provide cost-effective care.
• Coordinate care with resident, care providers, facilities financial services, and third-party payers.
• Oversee admissions and discharge activities.
• Coordinate referrals to and from the facility.
• Maintain positive relationships with residents, families, staff and professionals.
• Accurate charting and timely paperwork completion.
• Work independently and exercise sound judgment with physicians, providers, payers and residents/families.
• Communicate effectively to promote cooperation among residents, families, and staff.
• Maintain excellent time management in a high-volume environment.
• Maintain professional, respectful interactions with residents and staff.
• Perform clinical assessments, care planning, and direct coordination of medical services when required.

Education

Bachelor's Degree in Nursing or Social Work

RN license preferred; LVN or LPN acceptable

Two years clinical nursing experience preferred

Knowledge of Medicare, Medi-Cal and Medicaid programs

PCC Knowledge

CCM certification a plus

Continuing education/licensing

In good standing with Department of Public Health, License and Certification Division
