The Ohio State University Wexner Medical Center is seeking a Clinical Case Manager to facilitate patient care within a multidisciplinary team. This position focuses on efficient and compassionate inpatient care, ensuring a safe transition to downstream services.
Responsibilities include monitoring patient needs, collaborating with healthcare workers for comprehensive care, and proactively responding to health care trends. A Bachelor of Science in Nursing and RN license are required along with prior clinical and case management experience.
The Clinical Case Manager functions within the multidisciplinary team to facilitate an assigned group of patients through the health care system with the primary focus of efficient, effective, compassionate inpatient care and a safe transition to the next level of care. In some areas, this may include pre‑admission to post discharge coordination of care.
The Clinical Case Manager monitors the patient care process and physiological needs; documents, communicates, and collects data to evaluate and improve timeliness and appropriateness of care. The Clinical Case Manager collaborates with physicians, nurses and other health care workers to provide comprehensive case management assessment, treatment, and follow-up evaluation for patients and to communicate plan of care. The Clinical Case Manager maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization.
RN license and Bachelor of Science in Nursing required. 2 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required. Prior case management experience strongly preferred. This role must understand the health care continuum and have the ability to solve complex problems and possess strong communication and interpersonal skills. Must be able to work with various computer applications.
