Job summary

Staten Island JCC is hiring a Case Manager to support individuals receiving OPWDD services. The role involves developing individualized service plans, coordinating treatment, and advocating for clients. A valid NY State Driver's License and experience with the special needs population are required. Qualifications

• Must have a valid NY State Driver's License.
• 2 years’ experience working with the Special Needs Population.
• Knowledge of OPWDD Policies.

Responsibilities

• Oversees client care by assessing treatment needs and developing individualized treatment plans.
• Manages admissions of new clients by conducting comprehensive orientations.
• Coordinates treatment programs by developing schedules and overseeing service delivery.
• Tracks attendance and evaluates treatment effectiveness.
• Prepares comprehensive reports summarizing treatment outcomes and performance metrics.
