Qualifications

• Must possess CCM or ACM certification within entry period or be eligible for CCM/ACM certification.
• Licensure as required for one's discipline with active status in state.
• Bachelor's degree as minimum for licensed professionals; graduate degree preferred.

Responsibilities

• Coordinate with interdisciplinary team to establish discharge plan and contingencies.
• Guide treatment plans based on patient needs and preferences.
• Complete documentation and case management addenda timely.
• Monitor quality, timeliness, and payor requirements in patient care.
• Attend and contribute to weekly team conferences and care plan modifications.
