Job summary

Qualifications

• Bachelor's degree required.

• Ability to input data, perform calculations, and produce timely reports.
• Familiarity with community resources and coordinating services.

Responsibilities

• Coordinate the Person Directed Plan and monitor progress toward outcomes.
• Assess needs and develop interventions for individuals and families.
• Document activities and communicate with stakeholders.
• Travel within the service area as needed and maintain compliance with required screenings.
