Job summary

Baylor Scott & White Institute for Rehabilitation in Dallas, TX is seeking a Case Manager to join our interdisciplinary team and support clinical and psychosocial aspects of neurologic patient care.

The ideal candidate will have medical or rehabilitation setting experience, performing discharge planning, post-acute care planning, payer/referral management, and patient/family education with access to community resources.

Qualifications

• Current licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline.
