Job summary

Deblin Health Concepts in the Greater Houston Area seeks a Contract Case Manager to support LOC 3/4 clients, including school-based services, with local travel between campuses.

You will assess, develop and monitor treatment plans with clinicians, coordinate care, connect clients to resources, and document progress.

Requirements: Master’s degree in Human Services, at least two years’ case management experience, CPR, and English/Spanish preferred; ability to travel.

Qualifications

• Master's degree in a human services field required.
• Minimum of two years' case management experience required.
• CPR Certification required.
• Bilingual English/Spanish preferred.

Responsibilities

• Conduct client assessments and identify service needs.
• Develop, implement, and monitor individualized treatment plans in collaboration with therapists and other providers.
• Provide case management services to clients with Level of Care 3 and/or 4 needs.
• Facilitate coordination of care and connect clients with community resources and support services.
• Complete timely and accurate documentation, including treatment plans, progress notes, and coordination of care records.
• Provide individual and group case management services to students in school-based settings.
• Facilitate scheduled group sessions and provide ongoing support to students on an assigned caseload.
• Travel between assigned campuses and community locations as needed.
• Participate in team meetings, trainings, and other required organizational activities.
