Advocates in Town of Brighton, NY is seeking a Case Manager to document and maintain ISP records, health concerns, and support plans. The role centers on individual advocacy, empowerment, and helping clients achieve community integration.
The starting rate is $21.50/hour with weekday hours and occasional weekends as needed. The organization provides services for people facing developmental or mental health challenges.
Schedule: Monday through Friday 8am-4pm or 9am-5pm

The Case Manager is responsible for documentation and maintenance of program participants’ records and plans, including all areas of the Individual Service Plan (ISP), medical concerns and appointments. This role will focus on individual advocacy, empowerment, and community integration.
