Advocates in Framingham, MA is seeking a Case Manager to document and maintain participants’ records and ISP plans, medical concerns, and appointments, with a focus on individual advocacy, empowerment, and community integration.
This nonprofit, person-centered organization values respect, inclusion, and collaboration; the education requirement is a High School Diploma/GED (or Bachelor’s) with related experience, and a valid driver’s license with access to a vehicle to transport clients.
Starting Rate: $20.00/hour
Schedule: Weekday business hours, plus one weekend day (flexible schedule)
The Case Manager is responsible for documentation and maintenance of program participants’ records and plans, including all areas of the Individual Service Plan (ISP), medical concerns and appointments. This role will focus on individual advocacy, empowerment, and community integration.
