Advocates is seeking a Day Services Case Manager in Plainville, MA to design engaging activities, provide direct skills training, and support individuals toward personal goals. You’ll work within an interdisciplinary team, document progress, and collaborate with families to ensure high-quality, coordinated supports.
Starting rate is $19.50/hour with a Monday–Friday 8am–4pm schedule. Minimum education is High School Diploma/GED; an Associate’s degree or 2+ years in human services is preferred.

The Day Services Case Manager supports individuals in a day program setting by helping design and implement engaging, person-centered activities that promote independence, skill development, and community involvement. You will work as part of an interdisciplinary team to develop activity schedules, provide direct skills training, and support individuals in achieving their personal goals. This role also includes documentation, progress notes, and collaboration with families and team members to ensure high-quality, coordinated supports.
