Job summary

Advocates is seeking a Case Manager in Framingham, MA to document and maintain ISP records, support individuals’ empowerment, and promote community integration.

The role focuses on advocacy, empowerment, and person-centered planning with a starting rate of $21/hour. A high school diploma or equivalent, plus relevant experience, is required. A valid driver's license and reliable vehicle are preferred.

Qualifications

  • Bachelor’s Degree; or High School Diploma and two years’ relatable experience.
  • Strong interpersonal skills and ability to use good judgement.
  • Excellent written and verbal communication skills.
  • Basic computer knowledge.
  • Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients.
  • Strongly prefer a candidate with understanding of culturally diverse populations.

Responsibilities

  • Provide case management, skills training and support to individuals in areas including ADL, behavioral treatment plans, community integration, communication, health care, home maintenance and cleaning.
  • Participate in development of individuals’ ISP and program‑specific interventions; develop and monitor person-centered goals.
  • Assess individuals’ behaviors and complete daily data, monthly progress notes and quarterly reviews.
  • Assist individuals with obtaining all services and entitlements (housing, insurance benefits, etc.).
  • Foster community integration and support individuals to actualize this goal.
  • Participate in weekly staff meetings, supervision and clinical meetings as specified by supervisor.

Starting Rate: $21.00/hour

Schedule: Weekdays (Flexible Hours)

The Case Manager is responsible for documentation and maintenance of program participants’ records and plans, including all areas of the Individual Service Plan (ISP), medical concerns and appointments. This role will focus on individual advocacy, empowerment, and community integration.

Minimum Education Required

High School Diploma/GED
