Job summary

Omnicare Behavior Health LLC in Morris Plains and Brick, NJ seeks a dedicated Case Manager to establish therapeutic relationships, manage up to 20 clients, and coordinate care across health services. You will lead psychoeducation groups, document progress, and advocate for needed services.

Experience with mentally ill adults is essential. The role requires strong communication, organizational skills, and empathy in a fast-paced environment.

Qualifications

• Bachelor's degree in social work, psychology, sociology, healthcare or related fields; Master's degree preferred
• Two years of relevant experience in partial care or supervised residential setting
• Experience working with seriously and persistently mentally ill

Responsibilities

• Establish and maintain an ongoing therapeutic relationship and case manage up to 20 clients.

Qualifications:Bachelor’s degree or Master's degree in social work, psychology, counseling or a related field, and a minimum of two years of relevant experience in partial care or a supervised residential setting, working with the seriously and persistently mentally ill.

Responsibilities:Responsibility to establish and maintain an ongoing therapeutic relationship and case manage up to 20 mixed risk level persons served. Conduct various psychoeducation groups and provide individualized case management sessions. Maintain up-to-date person's served records, including weekly progress notes and develop individualized recovery plans collaboratively with persons served. Provide personal support; crisis intervention and advocacy for needed services and entitlements.

Skills/ Qualifications: Efficient oral and written communication, use of good decision-making abilities, conflict resolution skills. Recovery-focused approach to serving adults with mental illness.

The case manager will be responsible for assigning, monitoring, and organizing cases for social work. They will be directly involved in the lives of their clients, and provide resources and assistance. Planning health services, working with social workers and doctors, and scheduling treatment or recovery is all in a day's work. The case manager must be highly organized, empathetic, compassionate, nonjudgmental, and eager to help vulnerable adults.

Case Manager Duties and Responsibilities

• Accessing client needs, and developing strategies to help the client
• Providing support and resources for clients; good working relationship with support networks, government resources, and community resources
• Researching and referring client resources
• Respond to crisis situations that affect vulnerable or mentally unstable adults
• Evaluating and studying programs, services, and resources for quality and client needs
• Determining the correct course of action when helping clients
• Caring for the well-being of clients and helping a wide range of clients, including people with serious illnesses, addictions, disabilities or who have been neglected
• Advocating and raising awareness on behalf of clients and the needs of the community and local services
• Assisting clients by providing and facilitating various psycho-education groups that focus on clients mental health needs

Case Manager Requirements and Qualifications

• Possesses superb written and spoken communication skills
• Excellent interpersonal skills with colleagues, community leaders, policymakers and others
• Excellent time management skills; organized and able to prioritize
• Comfortable in a fast-paced environment with multiple cases
• Able to organize and manage necessary paperwork and documentation
• Self-directed and able to work without supervision
• Comfortable with building personal relationships and dedicated to helping others
• Empathetic and supportive with mentorship and leadership skills
• Bachelor's degree in social work, psychology, sociology, healthcare or medical and related fields; Master's degree in social work preferred
• Proficient computer skills, including Microsoft Office Suite (Word, PowerPoint, and Excel)
