Job summary

Prairieland Service Coordination, Inc. in Galesburg, IL is seeking a Case Manager / PAS & Crisis Coordinator for a full-time in-office position. You will complete PAS assessments, respond to crises, coordinate services, and advocate for individuals with developmental disabilities and families.

This role requires travel within PSCI’s service area, excellent documentation, and adherence to DHS requirements. Join a team dedicated to providing ethical, person-centered supports in a 27-county region.

Qualifications

• Bachelor's degree in a human services-related field is required.
• At least 1 year of cumulative experience providing direct services to individuals with developmental disabilities; experience may be from employment, volunteer work, or family caregiving.

Responsibilities

• Complete the PAS process per DHS PAS Manual requirements.
• Handle crisis-related activities, E-Packets, DHS forms, and documentation within deadlines.
• Assess needs and coordinate services through communication, linkage, monitoring, and advocacy.
• Advocate to ensure access to eligible and preferred services.
• Schedule and complete Discovery and Personal Plan processes as part of PAS.
• Maintain accurate files and documentation per DHS and agency guidelines.
• Provide problem-solving consultation to provider agencies following DHS guidelines.
• Complete daily billings and ensure accurate time reporting.
• Participate in staff meetings and trainings.
• Communicate concerns and service gaps to the Administrative Program Manager.
• Provide coverage across PSCI's 27-county area as needed.
