Job summary

OPARC in Montclair, CA is seeking a Case Manager for Creative Arts and Explore to coordinate person-centered services and develop PCP/ISP for about 45 participants. The role requires strong documentation, compliance, and collaboration with families and Regional Center partners. The position is regular full-time, with an 8:00 a.m.

to 4:30 p.m. schedule, Monday–Friday. Qualified applicants must meet DPS/CARF standards, possess a California driver’s license, and be prepared for background checks

Qualifications

• Must be at least 21 years of age.
• Associate’s degree in Social Work, Human Services, Business Administration, or related field, or a minimum of one (1) year of managerial experience.
• Knowledge of Title 17, DDS, Regional Center, and FLSA regulations.
• Organizational, communication, and leadership skills.
• Must possess and demonstrate a basic level of oral and written communication skills.

Responsibilities

• Manage a participant caseload of approximately 45 participants.
• Develop Person-Centered Plans (PCP) and One-Page Descriptions or use the existing PCP to guide the direction of the program.
• Based on the PCP, develop the ISP and other assessment forms in collaboration with participants and support team members.
• Coordinate, gather information for, and document meetings on a regular basis.
• Schedule meetings with participants and support team members to discuss and review progress on objectives.
