CASE MANAGER, CREATIVE ARTS AND EXPLORE

General Statement of Function

The Explore and Creative Arts Case Manager is responsible for coordinating and managing person-centered services for assigned Oparc participants through the development and implementation of Person-Centered Plans (PCP) and Individual Service Plans (ISP). This role facilitates ongoing assessment and progress monitoring and maintains accurate and compliant records in accordance with Developmental Disabilities Services (DDS) and CARF standards.

Case Managers are also responsible for delivering personalized, high-quality services through regular participant engagement and close interdisciplinary collaboration with families, care providers, and Regional Center partners, as well as providing advocacy, guidance, resource coordination, solution-focused support to remove barriers, and promoting skill development to help participants achieve greater success, independence, and alignment with their personal goals and preferences.

Essential Functions
Participant Services
  • Manage a participant caseload of approximately 45 participants.
  • Develop Person-Centered Plans (PCP) and One-Page Descriptions or use the existing PCP to guide the direction of the program.
  • Based on the PCP, develop the Individual Service Plan (ISP) and other assessment forms in collaboration with assigned participants and other support team members.
  • Coordinate, gather information for, and document meetings on an annual, semi-annual, and quarterly basis, or as needed.
  • Schedule meetings with participants and support team members to discuss and review progress on objectives.
  • Complete at least 10 hours per month of participant supervision support, including but not limited to front desk coverage, bus duty, and ratio support.
  • Review Community Coach daily progress notes to monitor participant progress, identify needed goal or tracking updates, and support accurate, compliant, person-centered documentation.
  • Complete Special Incident Reports (SIRs) in accordance with DDS/Title 17 regulations and submit to the Program Manager for review and approval. Complete Observation Reports and other required incident documentation as directed.
  • Maintain participant records in accordance with Oparc, Department of Developmental Services (DDS), and Commission on Accreditation of Rehabilitation Facilities (CARF) standards.
  • Support participants in addressing daily activity needs, personal concerns, and barriers to participation through guidance, counseling, and advocacy.
  • Serve as a liaison between care providers, Regional Center staff, Oparc staff, and peers to ensure effective communication and coordinated support.
  • Use participant feedback and satisfaction data to assess service quality, identify participant experience trends, and report improvement opportunities to management. Complete exit paperwork for participants no longer receiving services and conduct a six-month follow-up on their status from the date of exit.
  • Perform program intakes as assigned, develop recommendations based on assessment outcomes, and obtain and verify all required documentation, including employment forms (I-9, W-4) and validation of eligibility to work, such as government-issued identification and Social Security card, if applicable.
  • Facilitate Interdisciplinary Team (IDT) meetings as needed to assess changes in the individual’s condition, address emerging concerns, coordinate services, and ensure the program continues to meet the individual’s needs.
  • When applicable, transport individuals to and from worksites or community settings while strictly adhering to Oparc vehicle safety protocols, traffic laws, and best safety practices.
  • Adhere to the proper care and responsible use of Oparc-issued property, tools, and equipment.
  • Perform all other duties and responsibilities as assigned.
General Administrative Supports
  • Provide guidance to staff regarding outings and scheduled activities to ensure smooth operations.
  • Monitor and update the scheduling platform to reflect changes due to participant absences or adjustments in daily plans.
  • Prepare the scheduling platform for the following day, ensuring all activities and participant information are accurate and complete.
Employment Services (If Applicable)
  • Provide ongoing input to management to evaluate whether participant employment activities are appropriate, effective, and aligned with individual capabilities and program objectives, advocating changes as needed.
  • Upon intake, validate and complete work documentation (I-9 and W-4).
  • Provide benefits counseling and resources as needed.
Minimum Qualifications
  • Must be at least 21 years of age.
  • Associate’s degree in Social Work, Human Services, Business Administration, or related field, or a minimum of one (1) year of managerial experience.
  • Knowledge of Title 17, DDS, Regional Center, and FLSA regulations.

  • Ability to pass a Department of Justice (DOJ) background check and fingerprint clearance, complete a TB test and pre-employment physical, and provide proof of required immunizations, as applicable.
  • Valid California driver's license, proof of current vehicle insurance, a clear driving record, and two (2) years of driving experience.
  • Reliable transportation to travel and attend off-site meetings.
  • Must be able to work in outdoor environments, which can include, but are not limited to, allergens, excessive heat, excessive cold, excessive humidity, dampness or chilling, excessive noise, and/or slippery and uneven walking surfaces, as well as indoor warehouse-type settings that may lack heating or air conditioning and may have concrete flooring.
  • CPR, First Aid, and Crisis Intervention (e.g., CPI) certifications, or ability to obtain upon hire.
Preferred Qualifications
  • Bilingual in Spanish, ASL, or other languages.
  • Able to work in both indoor and community-based environments.
  • Able to support individuals during community outings, job coaching, and/or volunteer activities.
  • Able to lift up to 25 pounds independently and up to 150 pounds with one other person.
  • Assist with physical transfers, if necessary.
  • Able to push a wheelchair with a seated participant up to one (1) mile.
  • Standing, walking, sitting, pushing, bending, and pulling required.
  • Squatting and repetitive motion may be necessary.
  • Use chemicals in accordance with manufacturer's labeled instructions.
  • Adhere to the proper care and responsible use of Oparc-issued property, tools, and equipment.
