Job summary

LaSante Health Center is seeking a compassionate Case Manager to help patients address social, financial, and healthcare-related needs. The Case Manager will connect patients with community resources, public benefits, and support services while coordinating with the clinical care team.

The role involves assessing needs, developing service plans, and following up with partners to ensure access to services, with collaboration across therapists and providers in a patient-centered care model.

Qualifications

• Experience in case management or social services preferred.
• Familiarity with NYC community resources and public-benefit programs preferred.
• Strong communication, organization, documentation, and follow-up skills.
• Ability to work compassionately with vulnerable individuals and families.
• High school diploma or equivalent required.
• Bilingual skills are a plus.

Responsibilities

• Assess patients’ needs and develop individualized service plans.
• Connect patients with food, housing, transportation, benefits, homecare, legal, and other community resources.
• Assist patients with applications, referrals, and required documentation.
• Follow up with patients and community organizations to ensure services are accessed.
• Coordinate care with therapists, medical providers, and other team members.
• Document all patient interactions, referrals, outreach, and follow-up activities accurately and promptly.
• Track patient needs, referrals, deadlines, and progress.
