The HealthCare Connection (THCC) is seeking a Case Manager to support high-risk and complex patients. You will assess needs, build individualized care plans, and connect patients and families with essential community resources, guiding them toward improved health outcomes and quality of life.

Collaborating with an interdisciplinary care team, you will ensure coordinated, patient-centered care, coordinate referrals, and help patients overcome barriers related to social determinants of health,

Formación

• Bachelor’s degree required in social work or related field.
• One year of case management experience preferred.
• Experience with pediatric and adult populations is a plus.
• Familiarity with EMR systems and Microsoft Office is preferred.

Responsabilidades

• Identify at-risk patients and assess social determinants of health.
• Develop and coordinate care plans with healthcare teams.
• Facilitate referrals to mental health and substance use services.
• Advocate for patients and provide self-management coaching.
• Document services and track patient outcomes.
