Job summary

The LifeCourse Health Unit at the Boston Public Health Commission is seeking a dedicated case manager to support youth and families in reducing firearm violence and promoting health and safety in Boston. You will work with a team of case managers and partner agencies to provide intake, assessments, ISP development, referrals, and crisis support.

Qualifications

• High School diploma or GED required; BA/BS preferred.
• 2-3 years' experience with at-risk youth and/or histories of firearm activity (preferred).
• Minimum 2 years program experience in community outreach/social service/case management/education activities.
• Strong conflict resolution skills.
• Knowledge of Boston neighborhoods and ability to interact with high-risk individuals to diffuse conflicts.

Responsibilities

• Conduct outreach to identify youth and families for services as it relates to MEN'S HEALTH INITIATIVE.
• Work as part of a team of case managers across LifeCourse Health Unit programs.
• Support clients to prioritize short- and long-term needs and work toward ISP development.
• Provide client-centered holistic case management including intake, assessments, ISP development, referrals.
• Maintain accurate documentation and confidential client data while managing caseloads.
